General Wellness Health Assessment Visit
You will meet with one of caring providers who take the time to fully evaluate your health and perform basic A1C, Cholesterol, and Urinalysis etc. Some other tests in house if needed. If required then will discuss other health assessments with you.
Comprehensive Wellness with Blood Panel
The blood test panel provides important information about an individual's organ function, blood cell counts, thyroid function, and lipid levels. The blood test panel that includes CMP with GFR, CBC with differential, TSH, and lipid panel tests can provide a comprehensive assessment of an individual's overall health status. The lipid panel measures levels of different types of cholesterol and fats in the blood, including total cholesterol, LDL (bad) cholesterol, HDL (good) cholesterol, and triglycerides. High levels of LDL cholesterol and triglycerides and low levels of HDL cholesterol can increase the risk of heart disease and stroke.
The Ultimate fatigue panel
The fatigue panel consist of 8 different blood tests: These test may vary by age group. Some of the test include CBC, Comprehensive Metabolic Panel, Iron Total Binding Capacity, Total; Lyme disease, TSH, Urinalysis, Complete. The results of these tests can help identify underlying medical conditions that may be contributing to fatigue and guide further diagnostic evaluation and treatment. Fasting for 8 to 12 hours before a blood test is preferred.
Comprehensive Health screenings with annual physicals ages : 18-39 (both biologic sexes)
• Weight, Height, Blood Pressure: Annually
- Health History Screenings: personal hx and for known Fhx such as CV disease, diabetes, and cancer
- Eye exam: visual acuity annually; specialist funduscopic exam every 1-2 years (macular degen/glaucoma screening)
- Diabetes Screening: Annually with known risk factors; bi-annually if BMI is greater than 25
- Skin Screening: For suspicious lesions or moles; specialist f/u for biopsy if any

65 Years and Older : All the previous yearly tests will continue with these additions:
- Colorectal Cancer Screening: Baseline test if not done previously and continued tests based on baseline results
- Height and Weight: Annually as above (important as shrinking height could be a sign of osteoporosis)
- Fall Prevention Screening: Baseline and then as needed
- Depression Screening: Mental health assessment
- High Dose Flu Vaccine: Fluzone High-Dose (only available to 65+)
- Lung Cancer Screening: done between the ages of 55 and 74 and are a current or former smoker with a history of at least 30 “pack years” of smoking
Extra Screenings for Women
- Cervical Cancer Screening: Most women can stop having Pap smears if they have no history of cervical cancer. If a hysterectomy has taken place then no pelvic exam may be necessary.
- Osteoporosis Screening: Women are at higher risk for osteoporosis and will be screened more regularly at this age (Baseline at 65 years old, if not already established, and additional screening based on the results) (60 y/o if high risk)
Extra Screenings for Men
- Prostate Screenings: Continue annually
- Osteoporosis Screening: Every 5 years after the baseline is established (60 y/o if high risk)

Thyroid Panel
Thyroid Panel contains blood tests used to assess thyroid function. Thyroid Basic Panel contains blood tests used to assess thyroid function. The tests Free T3, Free T4, TSH measure levels of different thyroid hormones and the thyroid-stimulating hormone (TSH) produced by the pituitary gland.
Thyroid Ultimate
Tests included in the Thyroid – Ultimate Panel are related to thyroid function and can provide information about the health of the thyroid gland and the levels of thyroid hormones in the body. There are about 8 tests used to evaluate the function and health of the thyroid gland.: Reverse T3; T3, Free;T3, Total; Thyroglobulin Abs; Thyroid Panel (TSH, Free T3, Free T4); Thyroid Peroxidase Abs (TPO). Thyroid hormones are important for many bodily functions, including metabolism, heart rate, body temperature, and energy levels.
Express Cholesterol Panel
The Cholesterol - Comprehensive Panel measures different aspects of lipid metabolism and provide information on the risk of developing cardiovascular disease. See also our Lipid Panel.
The 4 tests listed - Apolipoprotein B (Apo-B), Lipid Panel, Lipoprotein(a), and Lipoprotein Fractionation, Ion Mobility - are all related to the measurement of lipids and lipoproteins in the blood, which are important markers of cardiovascular health.

The Heart Health - Comprehensive Panel is a combination of blood and urine tests that assess various factors that can affect cardiovascular health. These tests can help identify risk factors for heart disease and stroke, as well as provide information on potential underlying conditions that may be contributing to heart health issues.
